Long term care nurses treat patients with severe disabilities or chronic illnesses who require long term health care. Many long term care patients are elderly and live in assisted living centers and skilled nursing facilities. Most long term care nurses find jobs in these types of facilities, and some find employment in a home health care situation. The on-site long term care nurse is responsible for organizing a patients daily activities such as bathing, dressing, eating and taking medications. They are also in charge of monitoring their patients health, keeping their patients educated about their health conditions and planning for future needs. Long term care nurses establish an on-going relationship with their patients and provide a personal type of service not found in a hospital setting.
